An 80-year-old man was admitted to hospital with chest pain and shortness of breath. He had a long medical history of diabetes, ischaemic heart disease, and end stage kidney disease requiring regular haemodialysis.

An urgent chest x-ray showed a radio-opaque nodule—not seen in previous x-rays—in the right lower lung field.
