What is a Zambulance?

The Zambulance is an affordable transport option for remote areas in developing countries.

In several developing countries, people die from treatable diseases simply because they cannot reach health centers when they need it most.  Bicycle ambulances are a proven solution to this transport barrier in remote areas where current methods of transport are insufficient in speed, safety, cost, or availability. Since July 2005, Jessica Vechakul has been collaborating with Disacare Wheelchair Center in Lusaka to develop the Zambulance,  a bicycle ambulance that can be locally produced from common steel and bicycle components.

The Zambian Ministry of Health and the WHO have ordered over 50 Zambulances for health centers all over Zambia.  Independent home-based care and hospice programs have also orderedZambulances.  Within one month, over 50 patients were transport between home and health centers by a single Zambulance. Given the versatility of the Zambulances for people of all ages and health conditions, it has the potential to be a live-saving technology worldwide. 

The Stretcher: To facillitate transport of the patient through narrow passages, the stretcher is removable from the trailer.  The body of the stretcher is made from sheet metal because it is more durable and easier to clean than high-quality fabrics. The sheet metal’s inherent flexibility also provides suspension.  A larger chain secures the angle of recline of the backrest in place once the angle of recline is adjusted. A canopy  frame made out of rebar is welded to the stretcher’s backrest to provide a frame for a waterproof canvas to be strapped on as rain and sun protection for the patient. The Trailer: A hand tow bar, made out of rebar, allows for easy steering when the bicycle is not attached.  Sheet metal mudguards and fenders protect the patient from debris and the trailer wheels.  The low center of gravity makes this trailer more stable and safer.  The trailer can also be sold separately with a cargo box to transport goods. The Bicycle Clamp and Hitch: The hitch attaches the trailer near the bicycle’s rear axle by clamping onto the seat stay and chain stay.  The bicycle clamp is made only using welded angle iron, flat bar, nuts and bolts.  This simple bicycle hitch can accommodate for slightly different angles between the chain stay and seat stay of common bicycles. The nut and bolt hitch is a univeral joint, allowing for three axes of rotation: pitch, roll, and yaw.  Thus, the ambulance can travel on highly uneven terrain without off-balancing the rider or tipping the trailer.  If you download any of the materials on this site, please e-mail your name, mailing address, phone number, and information about your group to the e-mail address shown above. Gente descubriendo que dentro de los test de antígenos "solo hay una tira de papel", o que "el test da positivo" con agua de grifo. Creen que por eso los test son un fraude.

Esto es lo que no saben:

Esa simple "tira de papel" en realidad es una técnica de separación y caracterización que los químicos usamos con frecuencia. Se trata de una cromatografía.

Y sí, la cromatografía es supertecnología. Se basa en conocimientos fundamentales de química y biología que obviamente esta gente no solo desconoce, sino que se jacta de su desconocimiento.

Para los que tenéis curiosidad, en la tira cromatográfica de los test de antígenos hay una proteína que enlaza de forma específica con el antígeno del virus. Este enlace revela color. Eso es lo que vemos como "positivo".

La línea control contiene otra proteína diferente que enlaza de forma específica con otra que se encuentra en la disolución tampón. Esto sirve para controlar que el el test se ha hecho correctamente y está funcionando bien.

La disolución tampón en la que se pone la muestra tiene dos funciones:

(1) Sirve de eluyente para que por capilaridad la muestra impregne toda la tira cromatográfica. En cuanto la muestra alcanza la proteína específica de la tira, si hay virus se enlazarán a ella revelando color (positivo), y si no hay virus, al no haber enlace, se queda sin color (negativo).

(2) Sirve de tampón, es decir, hace que el pH sea estable. Esto es muy importante porque tanto la proteína de la tira cromatográfica como el antígeno del virus al que enlaza son muy sensibles a cambios de pH.

Si les cambiamos el pH, las proteínas van a cambiar de forma, a retorcerse. Como consecuencia, o no enlazan, o la proteína de la tira revela color por sí sola, sin enlazar con nada.

Esto también ocurre con cambios de temperatura, por eso hay que conservar bien los test.

La gente que está probando los test con zumos, agua de grifo, refrescos... está alterando el pH de la tira, y eso puede hacer que las proteínas se retuerzan y revelen color, como si fuese un "positivo".

Todo es cuestión de química, pero los que difunden estas tonterías ni lo saben ni quieren saberlo.

Los parámetros actuales para examinar el ritmo cardíaco y de la respiración de los niños -que a menudo se utilizan para decidir el tratamiento que se indicará- son inexactos y no están basados en la evidencia, según reveló un estudio reciente.

La investigación publicada en The Lancetindicó que, tomando en cuenta los rangos actuales, más de la mitad de los niños sanos de 10 años de edad pueden haber sido diagnosticados con signos vitales anormales.

Los científicos aseguran que los médicos han transmitido los rangos "saludables" a las generaciones de relevo durante años, aunque no se han basado en evidencia, por lo que será complicado cambiar este paradigma.

Sin embargo, una revisión de este tipo podría ser de mucha utilidad para los médicos.

Inexactos

La rapidez con la que un niño respira y la velocidad de su corazón son herramientas importantes para el diagnóstico de una enfermedad.

Por ello, el análisis se centró en 69 estudios de los signos vitales de 143.346 niños sanos de todo el mundo.

Aunque los científicos tuvieron que lidiar con la variación de los niveles de salud durante la infancia, el estudio mostró que -en promedio- la frecuencia cardíaca fue de 145 latidos por minuto en los niños de un mes de nacidos, frente a 113 latidos por minuto en los pequeños de dos años de edad.

Los autores produjeron nuevos gráficos que muestran los signos vitales saludables durante las diferentes edades y los compararon con los parámetros actuales.

Matthew Thompson, de la Universidad de Oxford, explicó que "los niños con frecuencia cardíaca rápida no están siendo detectados, mientras que algunos están siendo diagnosticados con un ritmo rápido cuando en realidad están normales".

Más pruebas

Rosalind Smyth, de la Universidad de Liverpool y el Hospital de Niños Alder Hey, dijo que este estudio debería conducir a más investigaciones que determinen el umbral entre los signos vitales normales y anormales.

Y agregó: "estas investigaciones conllevarán a una revisión de algoritmos, calificaciones de riesgo y parámetros para establecer estos límites".

Tales instrumentos necesitarán ser probados en distintos contextos y poblaciones antes de que puedan ser incorporados a la práctica clínica", concluyó Smyth.

Los daños hepáticos relacionados on el consumo de bebidas alcohólicas suelen hacerse evidentes cuando ya es demasiado tarde. Ante esta situación, un grupo de científicos de la Universidad de Southampton (Reino Unido) creó un análisis de sangre para ver los daños no aparentes en el hígado de las personas que beben alcohol en exceso. Según la BBC, los investigadores consideran que esta prueba –que usa el código del semáforo para mostrar el nivel del daño– será una herramienta valiosa para el diagnóstico temprano de aquellas enfermedades hepáticas que se manifiestan solo cuando están en niveles avanzados y el órgano comienza a fallar. 

Pese a que el hígado tiene la capacidad de regenerarse, cuando es sometido a un embate continuo el daño se vuelve irreparable. Además, las complicaciones son mortales.

El nuevo examen muestra los resultados en colores: el verde indica que es poco probable el daño hepático; el ámbar, que la probabilidad de daño es del 50%; y el rojo, que es muy probable un daño mayor y que la enfermedad es potencialmente irreversible.

De acuerdo con el estudio publicado en “British Journal of General Practice”, el análisis es la combinación de la prueba hepática que los médicos ya emplean habitualmente, con otros dos exámenes que miden el nivel de cicatrización en el hígado.

El análisis se probó en más de 1.000 pacientes y ahora es posible realizar una prognosis [conocimiento anticipado] de la enfermedad hepática.

Según los investigadores, entre los pacientes que mostraron un código rojo (50%) y que fueron sometidos a un seguimiento, cerca de 25% murió dentro de los siguientes cinco años. Sin embargo, ninguno de los que mostraron código verde murió o desarrolló complicaciones.

“Hemos visto que para la mayoría de los pacientes esta advertencia es un buen estímulo para dejar de beber o al menos reducir su consumo a niveles seguros”, dijo a la BBC el doctor Nick Sheron, quien desarrolló el análisis. Para Ian Gilmore, presidente de la organización Alcohol Health Alliance, “este estudio puede ser realmente útil para guiar al paciente hacia la atención especializada de forma oportuna”.

El análisis puede ser usado en personas con diabetes tipo 2.
